Four things set your real Part D cost

  • The formulary

    Every plan publishes its own covered-drug list. Off the list means full price or a formal exception request.
  • The tier

    Covered drugs land on tiers, from generics up to specialty. The same drug can sit on tier 2 with one carrier and tier 4 with another.
  • The pharmacy

    A preferred pharmacy usually beats a standard one on price. Filling the same script a mile away on Grant Road can change your copay.

How we actually price a Part D plan

    Write down every prescription

    Name, dose, how often you refill. Bring the bottles or a printout, since guesswork is where most comparisons go wrong.

    Tell us your pharmacy

    Whether you fill on Oracle Road, Ina Road, or use mail order, preferred status matters as much as the plan itself.

    Total the full year

    Premium plus deductible plus every expected copay, added up for twelve months, not judged by the monthly number alone.

    Check it again next fall

    Formularies and tiers reset every January 1. Last year's smart pick can turn into this year's overpay.

Watch out

Penalties, coverage gaps, and help paying

Go 63 days or longer without creditable drug coverage after you're first eligible, and Medicare tacks a late enrollment penalty onto your Part D premium for good. It looks small monthly and adds up over a retirement. It's one of the mistakes we see the most.

Lower-income Arizonans may qualify for Extra Help, which trims or wipes out Part D premiums and deductibles. Pima Council on Aging can walk you through the application; see our Tucson senior resources page.

Coverage from an employer or the VA may count

Creditable coverage through a job, TRICARE, or VA benefits usually keeps you penalty-free. Hang on to the yearly creditable-coverage notice your plan sends.

Advantage plans usually bundle Part D

Already on a Medicare Advantage plan with drug coverage built in? Don't add a standalone Part D plan on top. Doing so can knock you out of your Advantage plan entirely.

  • Ask about a generic swap before accepting a specialty tier

Medicare lets you spread your yearly out-of-pocket drug costs into monthly installments instead of a big hit at the pharmacy counter early in the year. It doesn't lower your total, it just spreads it out. Worth asking about if a January refill has ever stung. Enrollment goes through your drug plan, and we'll show you how.

Your yearly medication checkup

From October 15 through December 7, every Part D plan can rewrite its premium, deductible, formulary, tier placement, and pharmacy network. Your own prescriptions shift too. A plan that saved you money last January can quietly turn into one of the priciest options on the list.

Why isn't the plan with the lowest premium the cheapest for me?
The premium is a small slice of the story. Deductible, tier placement, and pharmacy status usually outweigh a few dollars a month in premium.
What if my medication isn't on the formulary?
You can ask your doctor to file a formulary exception, switch to a covered alternative, or pick a different plan at the next enrollment window.
What's step therapy?
The plan makes you try a cheaper drug first before it'll cover the one your doctor actually prescribed. Ask about it before you enroll if you're on a brand-name drug.
Is mail order always the cheaper route?
Often for 90-day maintenance drugs, but not always. We compare your local pharmacy against mail order rather than assume.
Can I pair Part D with a Medigap plan?
